Managing chronic conditions with confidence: Libcare's Chronic Medicine Management Programme

Living with a chronic condition shouldn't mean worrying about access to your medication. Libcare's Chronic Medicine Management Programme ensures members receive approved treatment for a wide range of chronic illnesses efficiently and affordably.

What's covered?

Libcare provides cover for:

  • 26 Prescribed Minimum Benefit Chronic Disease List (List A) conditions (eg, diabetes, asthma, hypertension)
  • 38 additional conditions (List B) (eg, depression, gastro-oesophageal reflux disease, osteoarthritis)
  • Approved medicine is funded up to the Generic Reference Price (GRP) and subject to an annual benefit limit for each beneficiary.

How to register

Your doctor must apply by contacting the Chronic Medicine Management team on 0800 12 CARE (2273). Supporting clinical information may be required.

Once approved, your treatment plan will be authorised. Any changes must be reported to make sure claims are paid correctly.

Where to get your medicine

You can collect your chronic medicine from:

  • Any pharmacy
  • Your dispensing doctor.

Submitting claims

To ensure smooth processing:

  • Submit claims within four months of service
  • Include the correct ICD-10 codes, membership details and provider information
  • Attach valid proof of payment for member refunds (EFT or receipt only) where you have paid upfront
  • Email claims to claims@libcare.co.za
  • Post claims to PO Box 653418, Benmore, 2010

Note: Late or incomplete claims may be rejected.
